Enrollment Forms & Waivers
For enrollment in any Horizon program, you must have an Enrollment form and Member Waiver on file. For your convenience, you may print them off from here and bring them in with you. To participate in any of our camps, fun nights, open gyms, community classes, etc, you must have a non-member waiver signed by a parent or legal guardian in order to participate. Please print out the form below and bring it with you.
